Dr. Shumaker & Associates is a preferred provider for: Delta Dental, Cigna PPO, Superior Dental, and United Concordia. We accept all insurances, so if you do not have one of Preferred Provider Plans, please call the office for more information. We understand that dental insurance and coverage can be confusing, so let us help you find the information you’re looking for!

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24-48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
